Required Paperwork for Sports at HFCS

(All paperwork and registration fee must be submitted to the office prior to

first day of sport in order for student to participate in practices/games/meets)

 

1.   Registration Form (each sport or camp)

2.   Sports Physical— Students entering 6th grade will need to have a sports physical that is good through their 8th grade year.  Students entering 1st-5th grade will need to have a well-child examination which is good for 2 years.

**Free sports physicals for 6th-8th grade at 2 locations!  7 a.m. – 7 p.m. – 7 days/week
